Yes, it does happen in Sick Day when he is infiltrating Drakken's lair. He bungies through the air duct and loses his pants at the bottom.

Does anyone else feel that this gag should be put to sleep? I can't be the only one. You've got my vote. Although, to be fair it doesn't happen as often as many people think. Look at this thread. We are struggling to come up with 12 times out of the entire series and a couple of them are ripped pants not lost. And to be fair Kim even lost her pants once, although there is no way they were going to show that one. But I have to admit it would be pretty darn funny if she lost her pants and had a pair of Ron's boxers on. Maybe a bit too adult for the show though, but it would be really funny.

Here's all the episodes: Monkey Fist Strikes Chrush A Sitch in Time (actually, he just loses his shirt that time) Hidden Talent Exchange Emotion Sickness Bonding Dimention Twist So the Drama (he loses them twice, another time we him without pants but he didn't lose them) That makes about nine times he's actually lost his pants on th e show.

Imagine if that happened in front of her parents. Uh-oh Ron, one-way trip into deep space.

Ron: Monkey Fist Strikes. Eagle rescue, boxers blue with pale blue dots. Crush. Bizarre grappling hook gun accident, boxers red with white spots. Grudge Match. Caused by Mr. Dr. P's magnet, boxers yellow with magenta spots. Car Trouble. Pants ripped fence-hopping, boxers light blue with dark blue spots Hidden Talent. Grappling bhook accident in Dementor's lair, boxers red with white spots. Exchange (twice) Boxers dark blue with dark blue spots, boxers blue with light blue spots. Sick Day*: Turquoise with light blue polka dots (bungie jumping into Drakken’s lair) Emotion Sickness: Shego incident. Bosers blue with white spots. Bonding*: Light Green with yellow polka dots (lion swipes them off while riding to Dementor’s lair) Dimention Twist: Sucked by video vortex. Boxers bluegreen with white spots. Gorilla Fist: Pants ripped by broken branch. Boxers blue with white spots. So the Drama: Blaster briefs are green with yellow spots. One presumes Ron's supposed to lose his pants to use these. Bueno Nacho HQ: Boxers turquoise with white spots. Club Banana dressing room: Boxers green with yellow spots, but no rocket engines this time.
*Edit: Thanks to Taechunsa, who contributed the underpants data for Sick Day and Bonding in his post below, which my list lacked. I've added it here to make the information complete.
Proof that, although Ron seems to wear his red and black shirt nearly all the time, he regularly changes his underpants.
Kim: Tick Tick Tick: she flashes purple bikini briefs tumbling behind the counter. Someone must have said something to somebody about this, because in all her subsequent tumblings while wearing a skirt or dress, somehow Kim's underwear always remains out of sight. What good this does when it remains in the opening theme to every episode, I don't know.
Low Budget: Kim's pants are removed by an aggressive alligator. Whatever is exposed remains offstage. Hey, I saw an episode of "That's so Raven" in which the title character loses her pants and is seen in underpants, so seeing a "de-pantsed" Kim isn't absolutely impossible.
